Sign-on Incentive:
Case managers will receive $500.
00 after employment of ninety (90) days and the completion of Provisional Certification; and an additional $1,500.
00 after a year of employment! Retention Incentive:
After one year, case managers are eligible for a $500 quarterly incentive equaling up to $2000 for the fiscal year.
Performance Incentive:
Additional monetary incentives available based on employee tenure and program performance.

Summary:
The job of a child welfare case manager is to assess child safety with the ultimate goal of keeping children safe.
A Case Manager seeks to keep children safely in their homes with their parents, and when unable to do so, they strive to safely reunify children with their parents whenever possible.
When reunification is not possible, the case manager's next primary goal is to ensure that children are safely adopted into permanent homes.
WHAT YOU WILL DO:
Conduct regular visits with children and parents in accordance with policy to ensure their continued safety and well-being.
Manage and monitor child safety and case plan progress from intake to permanency.
Provide service referrals to parents in a timely manner to facilitate case plan goals and permanency for the child.
Maintain monthly contact with all service providers and courtesy/secondary workers involved with the family and report activities and progress to the courts.
Refer children to medical, dental, vision, and therapeutic services and ensure follow up with recommended treatment.
Complete all required court reports and documentation and attend/testify at court hearings and proceedings.
Enter and maintain the case file in the electronic case management data system.
Requirements:
WHAT YOU NEED:
BA/BSW or MA/MSW; or Bachelor's degree in related human service field Direct experience in Foster Care/Adoption is preferred, but not required 24 months previous work experience WHO IS YFA? Youth and Family Alternatives, Inc.
